We realize how valuable your time is. We will make our best efforts to keep all appointments on time and to minimize your time in the chair. Please fill out the
following forms and bring them to the office completed to help expedite your first
appointment. Should you have current records at another office, please fill out the
transfer of records packet to obtain your most recent radiographs from your
previous dentist.

Records Release

We accept cash, personal checks, Mastercard, Visa, Discover, and
American Express. We participate with most major insurance
companies and file claims on your behalf. We ask that you stay current
with your insurance and employer information so that we can better
assist you with this process. For those seeking outside financing for
their dental care, we also work with Care Credit to provide zero
interest plans to help you obtain the smile you’ve always wanted.
