Cleanings apply to patients without periodontal disease. If periodontal disease is present, additional services and fees apply. This includes 2 additional cleanings at 3 month intervals. If periodontal disease is present and initial scaling and root planing is needed, those services would be at 15% off.

There are no copayments for the work, no claims to file.
No maximums, no deductibles.

Patients also receive 15% off the cost of any needed work:
Fillings, Root Canals, Crowns, Bridges, Partials, or Dentures.

Payment for all additional services is due at the time when services are rendered. Payment for the plan is required in full at time of first service.

This is not insurance, it is a discount plan. It is only valid at this office, and cannot be used with the a specialist, or any other promotions. If services are not utilized at 6 month intervals, the treatment is forfeited.
